CMS Sports Forms

The top three forms need to be completely filled out with student and parent/guardian signatures. 

 

Physicals must be completed after April 15, 2024 for the current school year (2024-2025) or the student-athlete cannot participate until an updated physical is completed. 

 

Submit all packets to Mrs. Haywood. If you don't have access to a printer, please email Mrs. Haywood and the student can come pick one up from her.
