Students are still required by the State of Tennesse to provide proof they have had a booster dose of tetanus-diphtheria-pertussis vaccine (known
as Tdap) by 7th grade.

All upcoming 7th grade students are required by the state of Tennessee to have a Tdap booster. Please check with your child’s doctor to see if he or she is still lacking this vaccine. Once your child receives the required vaccine, please bring proof to the school on the state-approved Immunization Certificate, which is issued from either the doctor’s office or the Health Department. If your child receives the vaccine somewhere other than the pediatrician’s office or the health department, the information still needs to be transferred to the Immunization Certificate. This documentation needs to be brought in either on or before August 1, 2022. If your child has a medical or religious exemption, this documentation is also required.
